How do I access my St. Claire HealthCare Patient Portal?

Where does my health information in the St. Claire HealthCare Patient Portal come from?

All the information in the portal comes from the electronic medical record that SCH maintains. This ensures that you have access to your most accurate, up-to-date information.

Can my family access my St. Claire HealthCare Patient Portal?

Yes. You can provide family members or portal proxies (legal designees) access to your Patient Portal. A portal proxy is a person that has been legally designated to view your medical records. To designate a proxy, you must complete an authorization form and provide legal designee documentation if required. Contact Health Information Management at 606.783.6570 to obtain your proxy consent form.

Is my personal information secure?

Yes. St. Claire HealthCare Patient Portal passwords are encrypted, and the URLs are rewritten so they cannot be copied and pasted. You and authorized family members or proxies are the only individuals who can access your portal. Also, a timeout feature protects your information should you leave your portal page open.

What do I do if I have problems with my Patient Portal ?

Simply login to your Patient Portal and click the “Contact Us” link at the bottom of the screen to submit your questions, or call 606.783.6570. Our system administrators will address any concerns you may have.
