Medical Records

St. Claire HealthCare understands that medical information about patients and their health is personal, and we are committed to protecting patient privacy. A patient's medical information is treated as confidential by the hospital staff and is available only to authorized users. The health record is the property of St. Claire HealthCare and shall be maintained to serve the patient and their healthcare providers.


Requests for medical records or medical information, including diagnostic images (i.e. x-rays, CT scans, MRIs, ultrasounds and echocardiograms) and laboratory reports, can be made through the Health Information Management (HIM) department. The department is located on the second floor of St. Claire Regional Medical Center. Department staff will assist the patient or legal representative in properly completing a Release of Information form. This form can also be downloaded, completed and returned to the HIM department in person, by fax or by mail. Requests are generally processed within 7 to 10 days. Patients are allowed to review and receive one free copy of each of their medical records sets.

Birth Certificates

St. Claire Regional Medical Center does not provide copies of birth or death certificates. These requests must be made to the Office of Vital Statistics. Applications are available at the Medical Center, and can also be downloaded from the Kentucky Office of Vital Statistics website.

Completed applications should be sent to the Office of Vital Statistics at the address listed below for processing, which can take 6 to 8 weeks. There is a $10 fee for each copy requested.
