Medical Records

Beginning June 29, 2026, most patients will no longer need to request imaging discs, as providers can use our new Epic electronic health record to securely access imaging studies electronically.

If you need a copy of your imaging studies on disc for your personal records or to share with a provider outside of UK HealthCare, UK St. Claire or UK King's Daughters, please visit the registration desk at the main hospital, located at 222 Medical Circle, Morehead, KY. Registration staff will notify the Radiology Department, which will prepare your imaging disc.

Patients should no longer visit Health Information Management (HIM) to request imaging discs.

St. Claire Regional Medical Center does not provide copies of birth or death certificates. These requests must be made to the Office of Vital Statistics. Applications are available at the Medical Center, and can also be downloaded from the Kentucky Office of Vital Statistics website.

Completed applications should be sent to the Office of Vital Statistics at the address listed below. There is a $10 fee for each copy requested and processing can take six to eight weeks.
