St. Claire Family Medicine Earns National Recognition for Patient-Centered Care

St. Claire HealthCare is pleased to announce that St. Claire Family Medicine Morehead-Downtown, Olive Hill, Sandy Hook, Frenchburg, and Owingsville locations have earned national recognition from the National Committee for Quality Assurance (NCQA) Patient-Centered Medical Home Program. NCQA is a private, non-profit organization whose mission is to improve the quality of healthcare and accredits and certifies a wide range of healthcare organizations.
The NCQA Patient-Centered Medical Home is a model of primary care that combines teamwork and information technology to improve care, improve patient’s experience of care and reduce costs. Medical homes foster ongoing partnerships between patients and their providers instead of episodic office visits. Each patient’s care is overseen by a provider-led team that coordinates treatment across the health care system. Research has shown that medical homes can lead to higher quality and lower costs and can improve patient and provider-reported experiences of care.
To earn recognition, the St. Claire Family Medicine had to demonstrate the ability to meet the program’s key elements and embody characteristics of the medical home. NCQA Standards are aligned with the joint principles of the Patient-Centered Medical Home established with the American College of Physicians, the American Academy of Family Physicians, the American Academy of Pediatrics, and the American Osteopathic Association.
