Kentucky residents interested in receiving the COVID-19 vaccine should complete St. Claire HealthCare’s vaccine eligibility screening form using the link below to receive vaccine availability alerts. We’ll ask some questions about your employment and risk factors to determine when you will be eligible to receive a vaccine.
When you become eligible, and appointments are available, you will receive an automated call with instructions on how to schedule your vaccination appointment. Please note the supply of vaccines is extremely limited, and appointments are not scheduled unless vaccines are available on site.

How do you determine who gets vaccinated first?
St. Claire HealthCare is following the Kentucky Department of Public Health’s phased approach to vaccinations. Everyone who properly completes our vaccine eligibility screening form will be placed on a waiting list and will be prioritized by our clinical vaccine advisory committee as vaccines become available.
If I complete the form today and I’m not currently eligible for vaccination, should I submit another form later when I am?
No. Duplicate entries may only delay the processing of your request.
