Diabetes is a complicated disease that requires daily self-management. Diabetes does not have to be a devastating illness. Education is the key to successful diabetes self-management.
The St. Claire Regional Medical Center (SCR) Diabetes Education Program provides comprehensive diabetes education for individuals with diabetes or who are at risk for developing diabetes. Our education program follows the recommendations of the American Diabetes Association. We are committed to giving our patients the knowledge and skills needed to successfully manage their diabetes so they can be healthy, feel better, and live a full life.
Diabetes Outpatient Services
The SCR Diabetes Outpatient Education Program is dedicated to providing quality care and service. Our team includes registered nurses (RN), registered dietitians (RD), and certified diabetes educators (CDE)--health care professionals with in-depth knowledge and experience in diabetes care. Since 1999, the SCR Diabetes Outpatient Education Program has been recognized by the American Diabetes Association for meeting and exceeding national standards.
Our diabetes educators understand that diabetes is a very personal disease. Your diabetes educator will discuss your goals and help you create a plan to meet your needs.
We offer the following services:
- Diabetes education for Type 1 and Type 2 Diabetes for all age groups
- Diabetes education for Gestational Diabetes
- Pre-Diabetes counseling
- Polycystic Ovarian Syndrome counseling
- Diabetes nutrition counseling
- Blood glucose meter education
- Insulin and diabetes medication education
- Community awareness services
Diabetes education at SCR is covered by Medicare, Medicaid, and most private insurance companies. There may be co-pay charges, depending upon your individual insurance plan. Individuals who do not have insurance may qualify for assistance. A SCR financial counselor, located in the SCR Patient Registration area, is available to help determine what assistance is available.
The SCR Diabetes Outpatient Education Program provides a free Diabetes Support Group the first Tuesday of each month. A variety of speakers discuss the many aspects of living with diabetes. Individuals with diabetes and their family or caregivers are invited to attend. For more information, call (606) 783-6702.
Diabetes Inpatient Services
During your inpatient stay, our diabetes educators and other health care professionals will work with you to provide for your basic diabetes education needs. Family members and caregivers are encouraged to participate in this process. We do understand that times of illness are not always the best time for education. If you need more information after discharge, you may be referred to our Outpatient Program.
To make an appointment, ask your provider to call the SCR Diabetes Education Program at (606) 783-6702 or (606) 783-6701
